Shoe Registration Required (sizes subject to availability):

– Register EACH CHILD with their name, not parent’s name (If you have 4 children, you will want to complete 4 separate registrations).
– Shoes are new or gently used and limited to one per child.
– Must be a pediatric patient of Central North Alabama Health Services, Inc., with an appointment in the last 12 months.

Distribution will be on a first-come, first-served basis.
